What To Do If You’ve Lost Your Vaccine Card

Still have your vaccine card? Good – but inevitably, some of us are going to lose or misplace it. When that happens, don’t panic – here’s what to do:
First off, check with the pharmacy where you got your shot – chances are they have your information on file and can issue a new card.
If that doesn’t work, check with your state’s immunization information system – while not every state has one, about half of them allow you to check the records online.
Finally, don’t contact the CDC – they don’t issue cards and won’t be able to help.
